Have you tried portsentry?
It has a run command option so you can run a script
to notify you or take action on the target and/or
port of the attacker, run traceroute, etc.

 or you can get precompiled binary at rpmfind.net
it's easy to setup, needs tcpwrappers and ipchains
installed.
