Couldn't you put all questions as documents (one each) into a single collection and have an array that indicates to which audit level / standard it belongs? Retrieve all documents that have e.g. "HIPAA" in this array and you get everything needed to achieve that level. You would need to generate all the data first of course, mostly linking questions between certificates...
