Frank, 
Bingo! I put an AL at exec 0 on Window Load to jump to
exec order 1000, and after redeploying, the form
finally shows up on browser!

I wasted a day chasing this. Thanks a lot. Now I need
to figure out which AL is causing the issue and how I
can rewrite it.
