I had the same problem and the wise listers were kind enough to help me out.
 
Simplest solution would be to make sure that the last column in the csv file has data for all the rows. You can re-arrange the columns in the csv file to achieve this.

I've seen that. For me it's always been because a comma was in the data or I had some hard returns where they should not be.  Sometimes opening the csv in a hex editor will give a clue.
 
Never have I had it work one time and not the other.

no problem. But every 50 or 100 records it repeats itself so I do it again
