This is the same error that I get when using database links to connect to a
remote database. The error only comes up when the remote database has been
shutdown. Because of the way that oracle handles database links the calling
database does not know that the remote link was taken down. So the Remedy
thread that called the remote database will get this error the first time
that it tries to connect after the remote was down. Unfortunately all of
the Remedy threads have to get this error and reconnect one thread at a
time. So this error can pop up long after the remote was brought back up if
there are long periods of time between calls to it.

We have seen this error intermittently on our development server and were
told the oracle database ran out of processes.
