The problem first manifested when I was importing a def file, but I also get that error message when I'm trying to create a new form in the admin tool (File > New Server Object, Regular Form, OK, ctrl-S to save (no modification to the form whatsoever)), which tells me its probably not the def file import at issue. (The def file does have the correct begin/end combo and imports successfully into our dev box).
Hmmm...
a new Regular form gives an error
a new Display only form gives an error
a new Active Link on an existing form works.
a new Packing list works.
I'm still sorting through what I found in the SQL log - thanks for the slap on the side of the head Matt :-)

Thad:
Look at the file with a text editor. The definition should begin with Begin schema and end with the word end on a line by itself. Can you send the file to me off-list?

Subject: ARERR 400 - form does not hold a form definition
**
Hello,
Recently, when trying to import a def file, we started getting an ARERR 400. After searching the archives, I found where someone else had the same issue, but I didn't see that a solution was posted. Based on that conversation, I tried just creating a new regular form, with no modifications and saving it. On save we get the following error:
The following item was not imported : xxxjunk (ARERR 55)
The file specified for a form does not hold a form definition (ARERR 400)
This is on our production server and doesn't happen on our dev server. Does anyone have any suggestions on where to start?
Server: ARS 6.3, no patch
Oracle: 9.2.0.6
OS: Win2k, sp4
Thanks,
Thad
