The file your looking for is in your install dir. You can see from your own log where it lives. The missing form is in this def file.

The data is in a file further down.

However, Tony had it right when he said you really need to go back to a backup and reinstall. Given the failures, this is only the beginning of your troubles... too many forms got failures. You can get more details on WHY the failures occurred if you dare to dive into the other (non-html) logs. Look in the one that has a suffix of .log and search for [ERROR] to see if the failures are true failures or just warnings. I'm guessing they're real since you're missing at least one form.

requirement are met, won't re-install.

Did you review the HTML logs when you installed ITSM7?  Are you sure all
the forms were imported properly?

If there were failures you should start over with a backup, and not try
and import individual items.

The logs should be in something like \Program Files\AR System
Applications\<server>\BMC Remedy Incident Management\Logs

Seperate directories and logs exist for each of the ITSM7 installs.

Anything red in the HTML logs indicates a failure.
