Your design seems good.
Have you checked the logfiles?
One guess is that the order of execution is mixed up. Or you may have
missed the Merge-execute-on-checkbox.
Why use SQL when you can do an ordinary Set-Fields-From-Form-Call?
I usually use $LASTCOUNT$ instead of checking tmp-fields. In this case, in
a form with very specific functionality, it does not really matter. This
has nothing to do with the functionality though...
