Ok.  Try this

Go into your ar.cfg (or ar.conf) file and verify that you have a line
Plugin-Log-Level:
In there...if not...add it...make it's value 400 like this
Plugin-Log-Level: 400
Now...turn on plugin logging in the admin tool and restart your remedy
server.

This will give you detailed messages regarding the startup of your plugin
server.  Look through it for any errors and report back

It appears that it is having problems with the Request ID, and how it is
virtually handeled.
