I don't use the BMC Approval engine, but I think I recognize this error. If you have an active link or filter with a set fields or push field action, there is a checkbox called Advanced. Clicking this allows you to set the server and form you wish to run against from values in data fields. The error you list is the one you get if the field that is meant to contain the form name to be queried is blank. The set fields/push fields errors out because it does not know what form to perform the operation against. So, my guess is that you have some bad data in your system. To track the bad data down, you could try to turn on filter logging to identify what filters are causing this error, then look at the filter in the admin tool to see what fields are being used to define the server and formnames, then search your data records to find entries that have these fields blank. Then delete or correct these records. One other thing to note is that the server crash may not be related to these approval errors - there is a time difference between the two events.
