I had a similar issue when we upgraded form 6.3 to 7.0 patch 1.
To resolve I had to delete the email forms, restart the server so Remedy would reload the forms and then redo the configuration. It worked fine after that. It appeared the upgrade did not like the forms there with data in them already.
