Hi Dorayaki,
The easiest way would be to login to both servers at the same time with
the same admin tool.
1. Then select the three fields
2. Do a ctrl-c (copy)
3. Open the form in the target server
4. Do a ctrl-v (paste)
5. Move the fields to the correct position
