This maybe another option. I have done similar workflow on ITSM 4 (later upgraded to 6) on the Change Tasking module for standard and emergency CRs but did not use the Approval Server (we were on a really old version of ARS at the time). We created tasks for approvals (and technical reviews) and had second form to house a matrix. Technical review tasks were created based on the system selected in the CR and if the CR was an emergency only the first 3 required approval tasks were created immediately (actually there was a 2 hour delay for the technical review tasks to be completed) the other remaining approval tasks were created later on a regular schedule (even though the approval was after the fact everybody was informed and signed off on the change).
