I've seen this when an xss checker on the web server would block
requests based on the parameters sent to the BackChannelServlet.  If
you turn use firefox, you can use the 'Live HTTP Headers' add-on to
view the parameters sent to the servlet.  You can then replay that to
the BackChannelServlet at your whim to find out why the request is
failing, checking your web server logs, servlet container logs, etc.

Do you have any security related add-ons for IIS or on the server in general?
