Open the form in the Admin tool

Click Form, Manage Views...

Click Properties...

Select the Appearance tab and change Web Toolbar to Hidden. 

Then refresh the cache on the Mid Tier and you should not see the
toolbar anymore.

        Hi everyone:

         

        I'm sure this is a piece of cake, but for the life of me I can't
find it.  I have a display only form that I display on a 6.3 Midtier.
The form is display only, yet when I pull it up using a direct URL, the
Midtier includes the Search bar at the top of the page...you know, the
bar at the top that tells you what mode you're in and gives you the
button to switch to new mode, etc.  How do I set it so that that bar
doesn't show at all?
