Yes, I am quite sure I opened the form in the Admin tool and saved it, but it 
didn't do any good.

But just to be sure, I tried it again- and this time IT WORKED !

PS- I am getting up there in years, so there is the remote possibility that I

I used to run the Remedy installation at UVA just down the road from
you!

 

Have you tried opening the form with the 7.0 ADMIN tool and then saving
it.  An older form format may be the cause of what you're seeing.
