You have to have the form ARDBC LDAP Configuration installed on your system. Is it? If yes, the Hostname is the hostname or IP address of your domain controller. The Distinguished Name is the username of the account you'll use to access the AD. Password is the password associated with that account.
Once that's configured, then create the Vendor form. When you create the Vendor form, it will display a list of available AD schemas on the far right. Pick the one that holds data you're interested in...probably the User schema. Once you pick it, add the fields you want. Then create the Request ID field by marrying it to the uSNCreated attribute.
