Andy, I don't really have a good answer for that.. except that it appears to be some sort of a software bug... I'd suggest you send your def file with your finding to Remedy Support and report your findings.. They might be interested to investigate it further with you..
Meanwhile just out of curiosity, did you ever update or redefine indexes on this form at some point and perhaps got a time out? Database timeouts seem to do a few funny things at times.. Do you have your arerror.log file over the period of time you might have done changes to this form? Might be useful to take a peek at what happened during that time. One way of correcting the definitions would be to delete all your indexes on this form, save the form, and recreate these indexes.. then take an export of the form and see if the duplicate indexes were removed.. I believe I will wind up just mirroring the production server and go from there. I use this server to test new features, then export the def files and import to the production server.
