Yeah the issue was the multiple companies on my system but when I create one of 
them original company and others Global issue is resolved for example

If you have X Y and Z companies and same users is using this system 

You only need to import the user on X company using Unrestricted Access option 
then others company the operational categorization will be -Global- , but in 
summary form select the companies as Y and Z.

 

But currently my issue resolved by select the Y and Z as Global operational 
Categorization

1. the incident must be assigned to a group when created.

 

are you using an incident template to create the incident ?

Do you have the right permissions? 

Do you created the login name one by one or bulk import?

 

Try searching the Login Name on User form and be sure it below to "General 
Access" and "Unrestricted Access".

I run a bulk import and this permission give me headache.

When saving a new incident I recieve the following error:
