You have opened up a can of worms.  I will try to answer one of you questions.  
You can have Approvers that have only Read license.  They do not require a 
fixed or floating in other words.  If you want them to be able to view the 
changes they will need Infrastructure Change Viewer or Infrastructure Change 
Submit depending on your goal.  For change you have several Group Permissions 
you can give that require license.  CM Dashboard User, Infrastructure Change 
Config, Infrastructure Change Master, Infrastructure Change Submit, 
Infrastructure Change User and Infrastructure Change Viewer.  The only ones 
that need a fixed or floating license is Config, Master or User.  I hope this
