If you rename the .csv file to a .txt file and then open it with Excel
and format the Request ID as text, all of the zeros will remain.  The
problem is that Excel sees .csv files as spreadsheet format and assumes
that leading zeros are unnecessary and unwanted

I agree with Shawn.  This exact same thing happened to me.  I had a
customer send me a .csv file with the Request ID field, but the leading
zeros were deleted from it (which happens if you open up a .csv file in
excel and make a change and save it).  So it ended up creating 150 new
records.
 
Make sure the leading zeros are in your Request ID field on your csv
file.

when you say you have mapped the Request ID field, what exactly did you
do? for the import to update, your import file has to contain the exact
Request Id numbers as in the form, or it will not work. If it is
complaining about field id 8 which is Short Description, which is a
required field, then it is trying to create new records, not update
them. The way I usually do it is to export the Request Id field out in a
seperate file, then cut and paste it into my import file, under field id
1.
