Rather than using the Run Process or $PROCESS$ PERFORM-ACTION-ADD-ATTACHMENT why not just do a Set Fields as Carey Black says. Don't do the PROCESS, just set Attachment field on your form with the value from the Attachment field on the EmailAttachment form. Or maybe I'm the one missing something here.
