That picture proves what the Remedy document said. So I think the reason is the difference between your WSDL and my WSDL.
After loding my WSDL, I didn't see the SOAPHeader element in the wizard, could you share your sample WSDL with me? Since the WSDL is provided by external company, I cannot change it.
