Earlier someone had mentioned they were having thread issues during an 
Incident install.  We just resolved some thread issues while installing 
CMDB 2.1.

Turns out that having Development Cache mode on doesn't play well with 
having Escalations disabled.  If Dev Cache mode is on, the server assumes 
that the escalation thread will be present.  Took BMC support two weeks to 
figure this out.  At least they finally did. 

During the course of this install we found two document defects that 
caused us problems:
 - Admin Only Mode - Docs say to turn it on for performance improvements. 
It needs to be off ( Defect ID SW00256289).
 - Escalations Disabled - Docs say to disable escalations.  If you have 
Development Cache mode on (another document recommendation), then having 
the escalations disabled will cause thread issues.

Just thought I'd throw that out there in case it helped anyone.

We're on AIX, but I don't think that was a factor.
