Actually, that's not right either. At a minimum it would have to be '<Attachment field>' != 'DB.<Attachment field>' AND 'DB.<Attachment field>' != $NULL$
But that will only check if somebody is deleting the existing attachment. It won't detect a change to the same attachment or even the replacement of the original attachment with a different attachment.
