Hello; Please let us know which version & Patch level, OS, etc you are running.
However a few things to check: Ensure that there is no one (human) accessing the mailbox, because I have seen where if someone marks a message 'read', it might not be processed by the email-process (this was in Version 5X) - if the mailbox has shared access you might need to check with the authorized people... Ensure that there are not any 'exchange rules' running for the mailbox, as they can interfere with the email-process accessing messages...
