I had a simular issue a while back.  Solution ended up being exporting
the form to def file and reseting the form type to display only.  It had
currupted in the save and was trying to set itself as a regular form.
Importing it back in overwriting original fixed it.  We tried a lot of
things first to correct the issue but even at the db level it would
error out.  I would suggest exporting this and a working display only
for to def file and comparing the settings.

Just a guess, but what would happen if you "exported" the Display Only
Form that is crashing (DO NOT DELETE the OLD FORM) - IMPORT the Display
Only Form and give it a new name and see if that one can be opened.
