Open the Application in the Admin tool and verify that the form CHG:ChangeAPDetailSignature is still listed in the list of forms for the application. I've seen this before where the form (different form, different application - same error, though) thought it was licensed, but for some reason it got removed from the application. Adding it back to the application fixed the issue for me.

Only deployable applications can be made licensable. Also, Forms have to belong to deployable applications to be made licensable. (ARERR 9864)." I'm not sure what could be the problem.
