Does the form have one or more tables on it that pull in data from other forms (or even the same form)? If so, make sure that the form it is pulling data from has an index that covers the qualification the table uses to pull data. I've also seen it take a long time where there is workflow configured to iterate over the rows in the table, and the processing took a while.
That's just one possibility. I would agree with the logging. I would also turn on Active Link logging.
