Environment:  ARS & Email & Mid-Tier 7.1 patch 4 on SQL 2005, with Helpdesk 
6.0, using authentication chaining - ARS-OS-Area

Licensed users logging in to mid-tier frequently receive the error message that 
they do not have access to forms that they have no problem accessing in the 
user client. The same user can click on the link 2 or 3 times and get the error 
and then click again and get complete access to the form in question. This 
behavior may or may not reappear in the next mid-tier session.

We were advised by our support people to restart the web services whenever this 
happens but it doesn't help.

I added the User form to the forms in the prefetch list but this does not seem 
to have made any difference.

Any suggestions would be greatly appreciated.
