This is weird. The field re-appeared as mysteriously as it disappeared. So I renamed it to "UserID" to avoid the naming problem. But when I tried to save the form I got "Field is not related to this form C56500229" (can't remember the exact wording). So I deleted the field, saved the form, then pasted the field back in, and saved again, and now everything seems to be working.
