Besides clearing all cache, both User Tool disk cache and the browser cache, try replacing the ar_wut.js file located in the User Tool Installation directory under the *lib* folder. On machines were the navigation-bar is working, pick up the ar_wut.js file and replace it on the machines were its not working. Seen instances were the same is not updated in few cases of upgrades.
