It really depends on what your problem is.. Are you having any issues with your plugin server? It's easy to tell this - if there are issues with the plugin server you will get errors on the screen. Or is the form being displayed with no errors but with blank fields?
I've had both kinds of problems in the past of different occasions.. When you get an error on your screen, it usually is the plugin server which either fails to start or the AR Server cannot connect to the plugin server.. We'll need to know what error you see on your screen if this is your problem.. If you are faced with just a screen full of blank fields with no error, then chances are your workflow has not got imported. Look for the def files for this workflow in the templates directory of your installation. I think the file name is something like ServerInformation.def.. If that's not the name I apologize, but you should have no problems locating it, the name is pretty intuitive..
