I had a similar issue where a particular support group was "sometimes" not 
receiving email notifications. Turned out that that particular support group 
had "Business Hours and Holidays" setup, none of the other support group had 
them. 
This may not be related to your issue as restarting the EE fixes your issue. 
When you say " notifications are intermittently not being created".. you mean 
in the AR System Email Messages form, correct? 
Check form NTE:Notifier Log. Record Identifier = INC00000xxx. To make sure your 
notification is being created.
