Just to add to Frank's suggestion, create a group called InternetAccess or
something like that, and make that user a part of that group, and keep that
user access to that form only and nothing else.. Also disable guest user if
you do not need guest users enabled for whatever reasons.. This will make
that user secure, meaning even if people do figure the username and password
of that user (the username and password has to be passed in clear text in
the URL), they will not be able to take undesired advantage of that..
