Does the same thing happen when you log on as an Administrator from the web?

I've created a custom form and in the User Tool all fields display and all
workflow functions as expected. However, when the form is accessed from the
mid-tier only a few of the fields display. I've flushed the cache several
times and applied view\change permissions to all form objects without
success. This not a layering problem and I have same issue in Firefox.
Logging from User Preferences has only noted what was successful and nothing
with regard to what was not displayed. Mid-tier and arerror logs have not
recorded anything obvious.

Any thoughts on why only a subset of fields would display when a form is
viewed from the browser?
