What exactly are you looking for? I’m not sure how much help will a description 
of the T table and the Helpdesk form view will help you if there have been 
alterations / customizations done to this form which should be the case at most 
sites.. You could potentially get 10 different sets of information from 10 
different people..

If you are looking for a virgin OTB install, I could perhaps give you that next 
week, after I setup a new server..

Even with that, the data types could differ depending on whether the choice of 
database character set type was UNICODE or non-UNICODE... for e.g.. varchar on 
non UNICODE, vs. nvarchar on a UNICODE database
