When you are asking for help it helps to explain the purpose and who you
are, not just a request (that quite frankly is meaningless) and anonymously
as bmcremedy12.

Even if you don't have access to the database you should have access to the
Developer Studio or Administrator tool or be able to talk to someone who
does?

If not I don't know what you need the information for, unless you are
dealing with the API and don't want to admit to others that you don't know
what forms you need.

If you are doing API work for an integration no one would reasonably expect
you to know all the forms you need unless you represented you were an expert
on Remedy and aren't, so just ask someone that has access to the tools at
your site.

Which forms are there comes in dozens of variations depending on what has or
has not been installed, and depends on the version, no matter what
interpretation of that you meant.

Ps. Yes I am a bit cranky today J
