Is the web service you are trying to consume exposed from Remedy or external tool? From the screenshot, it doesn't seem to be exposed from remedy.
If exposed by remedy, then username and password tokens should have been there by default. Would you be able to use a freeware like soapui and try consuming the web service and see if you don't get an error? Using this tool it would be fairly easy to change mappings and troubleshoot.
