Bryan, Ensure that the host you requested the license for is the same one that shows up in the upper right corner of the 'Add License' window...I've had problems in the past with virtual hosts changing mac between boots and such, so it's possible that it changed, if that's the case, you will need to ensure to get a new one with the new host id, and then work with your virtual team to ensure the mac id doesn't change on you.
