That's been our experience as well. If we don't flush the Mid Tier cache we have no issues, but if we do, there is still a subset of people that will get "caught exception" and HTTP 500 errors. Fortunately, we're slowing down our need for 8.1 fixes so we should be doing less code changes to ITSM 8.1 going forward. Hopefully before our next big custom activity BMC gets the Mid Tier completely fixed.
