I think the point that Florian is trying to understand is whether this
is a problemwith adoc-mode.el or an issue of the font that you are
using. 

You could try this. Type M-x list-faces-display

This should show you all the faces that adoc is using for titles, named
adoc-title-0, adoc-title-1. If they are all the same size there, then
they are going to be also in an adoc-mode buffer. In which case, it
might be a generic emacs problem.

Next step would be to try a standard document, to see whether the
behaviour is consistent for you and someone else.
