Yes <listitem><simpara><?dbfo keep-together="always"?>Lorum ipsum</ simpara></listitem> could be made to work with some tweaking of the XSL stylesheets.
At least using +++<?dbfo keep-together="always"?>+++ the PI can be inserted on paragraph level without impact on the list structure. I completely forgot about +++ and tried pass:[] before which failed.
