Lex, you were right! I had edited what was originally a HTML file to
AsciiDoc-ise it and had failed to convert these links. :(  Now that I have
corrected them the document now converts correctly to HTML using the 'a2x'
tool.

Now my adventure down the AsciiDoc road can continue.
