The issue arises because the AsciiDoc is backend agnostic, it has no knowledge
of the syntax or semantics of the backend format that it generates (this is by
design). It treats URLs just like any other macro call, so if they include
double-underscore quotes (constrained emphasis) they will be substituted unless
they have been escaped.
You could disabled them altogether if double-underscore quotes in URLs is
something that you use a lot e.g. with:
:quotes.__:
