Is there a way to suppress the creation of a separate primary index term for each secondary and tertiary index entry? I noticed that in creating an index, that (((first-term,second-term))) provides this docbook:
<indexterm> <primary>first-term,</primary><secondary>second-term</ secondary</indexterm> <indexterm> <primary>second-term</primary></indexterm> For my purposes, it clutters up the index to have all the secondary index terms separately listed by themselves.
