How can I print ++ in monospace? I have tried every combination of
escaping I can think of:

* ++\+\+++
* ++\++++
* +++\+++
* ++++++
* +\+\++
* +\+++
* ++\++
* ++++

and none of those render correctly.

This is described in: http://www.methods.co.nz/asciidoc/userguide.html#X77

Try:

* +pass:[++]+

or

* +$$++$$+

I know it looks silly ;-)

PS My other suggestion would have been:

* ++++++++++++++++

but that does not do what I expected, and is even more silly ;-)
