Tomek, When you ask questions here please always specify the version of asciidoc and the backends and toolchains you are using. Styling of the docbook indexes is dependent on the toolchain you use. See FAQ http://www.methods.co.nz/asciidoc/faq.html#_sources_of_information_on_configuring_docbook_toolchains for information on toolchain configuration.
