The (((...))) index entry macro (indexterm) doesn't actually display any
text at the point it's entered, it just marks the spot and displays the
text in the index.

You also need an 'index' section (see this example

Note also that index entries are generated by DocBook toolchains (HTML
outputs don't generate index entries.
