I would look to doing this in CSS.

I think I lot of people that use AsciiDoc aren't thinking about the fact
that CSS can handle most of the layout and styling concerns if you step
away from the asciidoc.css stylesheet and embrace your own. With CSS3 you
can do very intelligent styling such as nth-of-type, nth-child, etc, etc.
